What the wording tells us
"A total of" is the only accumulating qualifier among the fifteen conditions, so the natural reading is that cigarettes add up rather than being counted within a round.
This is also the only one of the fifteen that does not name a simulation. Pairing it with the smoking simulation rests on two different levels of evidence: the developers' reveal announcement describes a simulation about smoking at speed, and three independent sources agree on the name — but no official text states it.
